[WardFive] FW: Community Health Assessment Kick Off Sept.25, 11am - 1pm

FYI - Forwarding an invitation and a flyer from the Washington Hospital Center (WHC) about an upcoming Ward 5 Community Health Assessment. Results from past assessments were shared during an Ward 5 Advisory Council community meeting and revealed that Heart Disease, Diabetes and Obesity were three major health concerns for the Ward.

WHC located in Ward 5, uses results from the health assessments to help determine where to put resources to address the needs of the community. Spread the word!
