[WardFive] RSVP: Religious Traditions Outreach Initiative/MedStar's Patient Support Network

Dear Ward 5 Community; 
 

You are cordially invited to the campus of Washington Hospital Center to discuss our recently instituted Religious Traditions Outreach initiative.  Most especially, we will use this occasion to work together to develop our Patient Support Network.  (See attached.)

 

In order to accommodate everyone, two separate meeting times have been setup.  (See below.)  Both meetings will be used to develop the Patient Support Network.  Thus, you only need to attend one.  Please RSVP to me by February 8th as to which meeting you are able to attend. 

 

Meeting A: February 20th from 4:00-5:00 PM [POB South – Conference Room C]

 

Meeting B: February 27th from 5:00-6:00 PM [POB South – Conference Room C]

 

 

The goal of these meetings is to produce a training agenda & schedule for those members of your congregation who wish to participate in the Patient Support Network

 

When we get closer to the date(s), I will be sending out an agenda for the meeting along with maps and directions.  I look forward to seeing you all soon.

Dear Ms. Steptoe,

 

It was a pleasure to meet you today.  Let me introduce myself again.  I am the Community Outreach Coordinator here at Washington Hospital Center (MWHC).  I was brought in to handle this newly designated role in order to build bridges between the Hospital Center and the District in an effort to promote public health.  I wanted to reach out to you and let you know about a new initiative established by Washington Hospital Center.

 

My first project is the MWHC Religious Traditions Outreach.  Through this initiative we intend to create programming that serves the health needs of religious communities across the district by partnering with various churches, synagogues, and mosques.  Our first program currently in development is the Patient Support Network.  This program will empower members within a religious community to aid individuals who have recently been discharged from the hospital to follow their designated health plans. (See attached document for more details.)

 

We have two meetings scheduled for later this month to discuss how we can best execute the Patient Support Network in a way that will work best for the community.  Leaders from various congregations across the city have been invited to participate.  If you are available, we would be most interested in having you attend and share your perspective on how a program like this can succeed.  [And please feel free to share this invitation with anyone who may be interested in attending.]
